Abstract

Dissecting aneurysm of the aorta, more recently known as aortic distio is a highly lethal COflditiofl. 1 With the availability improved diagnostic modalities, medical and surgical therapy since 1960, the survical has improved significantly.23.  Hc was started on antihypertensive trcat ment to which he responded well. However, after one year he was operated on at Toronto ()fl November 27, 1981. The diagnosis of Aortic dissection ascending aorta (Type II) was conf irmed. He underwent aortic valve replacement and ascending aortic graft with reimplantation of both coronary arteries.  Discussion: Najafi uses the term Anterior fir the proxi- mal and posterior for distal dissections. As shown figure 1(A) the intimal tear is very well seen just distal to the aortlc valve and dye is seen entering into the huge, dilated false lumen. The results of surgery depend upon acuteness of the process and replacement of the aoitic arch versus replacement of the ascending aorta. In the series of Cachera et al 9 there 0% hospital mortality for stable Pat1C1tS while 24% fbr acutely ill patients. The presence of shock, tamponade, C.V.A. or renal failure diti not correlate with hospital niorta1it 1 0
